Supporting people with learning disabilities and/or autism
Improving customer lives with community-based clinical and care services and housing solutions.
All about the service
This service supports people with:
- Learning disabilities
- Mental health issues
- Behaviours of concern
Customers live fulfilled and rich lives in their own homes, in their chosen community. Customers are supported into the community from in-patient or long term institutional settings. This includes the transition from residential schools.
Helping customers achieve their aspirations
Find out how Tyneside and Northumberland learning disabilities services are helping customers reach their goals.
A range of service models and pathways to achieve this are available, including:
- In-reach and discharge support
- Supported living in shared and self-contained accommodation
- Residential care
- Emergency respite and short term services
We do not replicate the institutional or clinically orientated environments but create joyful, lively and characterful places people want to live.
