Upon receipt of your referral/application form, we will contact you within 3 working days to arrange a visit to carry out an assessment of your needs. If appropriate you will be appointed a Project Worker, who will work with you on a Support Plan to identify objectives which will enable you to work towards developing and maintaining your independence.
